Respected Sir/Ma’am,
I am writing this letter to express my concern regarding the incorrect test results provided by your laboratory. I got my tests conducted at your lab on 22 May 2026 under Patient ID/Registration No. 510894653
The reports issued by your laboratory showed incorrect findings, which caused unnecessary stress, confusion, and inconvenience. After consulting another doctor/laboratory, it was confirmed that the results provided by your lab were inaccurate. Such errors can seriously affect a patient’s treatment and health decisions.
I request you to kindly investigate this matter thoroughly and provide clarification and full claim regarding the discrepancy in the reports. I also request appropriate corrective action to ensure such mistakes are not repeated in the future with the compensation amount for the stress happening in the family.
Please treat this matter seriously and respond at the earliest. I have attached copies of the reports and related documents for your reference.
