Dear Sir,
My mobile was lost with sim card which I was using last 12 years continuously. I have submitted all necessary documents as per requirement of nodal officer like last recharge date,amount, copy of sim card purchase with sim number, mobile model no. Type of recharge everything. After stolen my mobile I come to know that the name of sim holder mismatch with me which is using last 12 years continuously. I have contact through mail with WB nodal officer, but no solution come from him. Although I have refunded my mobile set by Kolkata police after doing a FIR.
