This is to inform you that Mr. Manmohan Singh S/o Lt. Makhan Singh, Age/sx@- 55yr/Male, R/o Plot No- 02, B. No – 29, Nehru Nagar East, Bhilai, Dist – Durg, was admitted on 31.01.2021 in Sparsh Multispecialty Hospital on 31.01.2021. An amount of Rs.50000 was submitted as an advance payment vide Receipt No: 202121978. The patient had been treated under Pradhan Mantri Jan Aaroraya Yojna (PMJAY) for ESIC employee benefit. The whole treatment was done through cashless means under PMJAY. Hence, the Advance Payment had to be returned back to the Patient or his/her family member.
The payment of Rs.50000 have not been reimbursed till date.
